ADVOCATE MDs CLIENT SERVICES AGREEMENT
This Client Services Agreement ("Agreement") is made between AdvocateMDs, a physician-led healthcare advocacy firm, and the undersigned Client.
I. PURPOSE AND SCOPE
AdvocateMDs provides physician-led guidance to help clients navigate the modern healthcare system. Services include explanation of medical information, guidance on treatment options, communication support between providers, and navigation of healthcare facilities.
II. NON-TREATMENT DISCLOSURE
AdvocateMDs does not provide medical treatment, diagnosis, or emergency care. The services provided are advisory and administrative in nature. Our advocates are not your treating physicians, and no physician-patient relationship is established for the purpose of medical care.
III. CONFIDENTIALITY
AdvocateMDs agrees to protect the privacy of all medical records and personal information shared during the course of the engagement. We will not disclose medical information to third parties without express authorization.
